It's a Team Fortess 2 joke. You've got a Spy class which can disguise theirselves as an enemy team member. As you can see, the opposing team member was HEALING him, so he sprayed that on the wall, gave the medic a GOOD look at it, then killed the Medic.

Believe it or not, that video actually SPAWNED an achievement for the Medic.
